Administering an assessment is the first step in building the Care Manager's relationship with a loved one and the family. The Care Manager conducts a comprehensive psychosocial assessment to understand your loved one's physical, cognitive, emotional, social, and spiritual needs better. Building trust with a loved one is essential when determining a care plan that includes their past life, hobbies, likes and dislikes, and favorite foods. A home assessment will decide if aging in place is appropriate to ensure the home is safe, reducing falls and increasing the quality of life at home.
A Care Manager oversees and supports care, coaches, and mentors family members and caregivers, and makes scheduled home visits to ensure all your loved one's needs are being met. They provide progress reports and care plans, communicate with an appointed family advocate, and advocate for their loved one.
A Care Manager helps their client navigate the medical system and sets up a care plan that maximizes their client's health and well-being. As an advocate, the Care Manager fights for their clients, requests appointments, assists with medication management, sets up transportation to office visits and PT and OT appointments, is available for medical emergencies, and plans nutritious meals and appropriate exercise programs.
A Care Manager has extensive knowledge of and access to appropriate resources best suited for their families. They consult with family members and their loved ones to initiate services and programs available in the community. Orange County has a wide range of community resources, including Adult Day Health Centers, Senior Centers, Community education, support groups, and benefits provided by Social Services.
Considering the competitive market, finding an affordable care home and the most appropriate level of care for our loved ones is incredibly difficult. There are assisting living, independent living, memory care within assisted living and out, nursing care, and board and care homes. Not only is this head spinning, but finding the right price that fits your budget and provides the appropriate level of care will stop you in your tracks. A Care Manager has relationships with experienced placement agents and personal connections within the community. A Care Manager is a God send who determines where to go and why.
Home healthcare that provides home caregivers is also an option. Ninety percent of seniors want to stay home. A Care Manager would do a thorough home assessment to determine if home care is appropriate, what social supports are available to help avoid isolation and depression, and whether home care is affordable. A Care Manager takes the guesswork out of the decision to place and give private care.

I'm a 62-year-old Orange County native, a cancer Thriver, married to my love Tobin, and we have four grown children. My husband and I live in Irvine with our three cats and puppy dog, Shimmer. We attend Calvary Church in Chino Hills and love going to the movies and playing Pickleball.
Community service and volunteerism are key to understanding the issues that truly effect all of us. I am passionate about educating my community about the issues affecting the older adult community, from the onset of Alzheimer's disease to advocating for seniors in medical and community support services. The system of care is nearly impossible to navigate; I have spent years navigating through the best resources in Orange County. Once people discover I am a Gerontologist and Geriatric Care Manager, they always have questions about a family member or neighbor needing care. Orange County is rich with support services, and I am committed to getting these resources into the hands of families. I am a Community Outreach Ambassador for Alzheimer's Orange County, which keeps me busy with Community education, resource fairs, meeting with assisted living communities, board and care homes, memory care, and nursing facilities, as well as meeting with the leading experts in Geriatric, Palliative, and Hospice care.
I am a fighter, critical thinker, and problem solver. I have the experience and expertise to help families make tough decisions. I move through the process with a calmness that eases the stress and anxiety of families. I am a much needed resource for today's senior community. We all need a team of support in our lives, including a great lawyer, an integrative doctor, an honest mechanic, and a trusted advisor when making tough decisions for our loved ones, especially when we have to have the uncomfortable conversation about end of life care. I have crisis management, counseling and family intervention experience that is essential to help all family members come together to build consensus about what is best for their loved ones. I wish I would have had a Life Care Manager to help me during the hard times.
